Patients are seen by appointment
We ask that you call the office to schedule your child's visit, but
please be assured that it is our policy to work in sick children on
the same day of your call. When you call our office for an appointment,
the receptionist will ask you questions regarding the reason for the
visit. This will allow the receptionist to set aside sufficient time
for your child to receive the care he/she requires.
Multiple
visits
If you have more than one child who needs to see the doctor, we ask
that you make separate appointments for each one. This allows us to
ensure that each child receives the attention that he/she deserves in
a timely manner.
Visit times
Please advise us if you believe your child has a contagious disease, so that we can make special arrangements if indicated when you call.
Missed
appointments
If you must miss a scheduled appointment for any reason, please let
us know in advance - we really appreciate 24 hours notice. This allows
us to offer your appointment time to someone else.
Late Policy
That if you are more than 15 minutes late for your scheduled appointment time that you will be asked to reschedule
Consent
for treatment
When someone other that the parent or legal guardian is accompanying
a patient, we must have our office consent form signed prior to the
visit. The form grants permission for another specific person to bring
the patient in for treatment. That person must be at least
18
years
of age. When your child comes in for immunizations, a parent or legal
guardian must be present to give consent.
Medical
forms
Patients needing sports cards, work permits or camp forms must schedule
an appointment to be examined. Please have your portion of the form
completed upon arrival in our office. We recommend scheduling sports
and school physicals early in the summer. This will allow adequate
time
if further tests or evaluations are necessary.
Convenience
We maintain two offices, one in Chillicothe and one in Waverly. All
of our physicians see children in both offices on a rotating basis.

Adena
Pediatrics is a participating provider in most insurance plans.
Please be sure to ask if we accept your insurance.
If your insurance
requires a co-payment or deductible, it is our policy to collect
that
fee from you at the time of your visit. For your convenience, we accept
cash, checks and most major credit cards.
Failure to keep
your account current will result in denial of routine appointments until
the account is paid in full, though we will see patients on an emergency
basis.
We recognize that
medical bills can sometimes be a burden. If this is the case, please
talk to us. We are aware of several sources of public and private funding
that may assist you. Depending on your income level and other factors,
you may be eligible for health insurance throught the Healthy Start/Healthy
Families program from the State of Ohio. Please feel free to speak with
us - we want to make it possible for your children to receive the health
care they need.
