Sponsorship Request

As a not-for-profit healthcare organization, Adena Health System is committed to the health of the people in our communities. We receive many requests every year from schools and organizations seeking sponsorship funds and other assistance. If you have a request that will help improve the health of the people in our communities, we ask you to complete this form by answering all questions and filling in all fields. Please allow a minimum of two weeks for us to evaluate this request and get back to you.
