Your Appointment
We accept patients by referral only. Bring your insurance card, all related medical records, reports, the name and address of your referring physician, a list of all medications you are currently taking and original diagnostic films like x-rays or MRI results with you to your appointment. Also bring a valid driver’s license.
Insurance and Workers Compensation
Adena Pain Management accepts most insurance plans, including Medicare, Medicaid and Bureau of Workers’ Compensation. If your insurance requires a referral or pre-authorization prior to a specialist visit, please make arrangements before your visit. Many insurance plans that require a referral or pre-authorization will not pay any of the cost of treatment if you do not have a referral or pre-authorization.
If your services are to be billed to the Bureau of Workers’ Compensation, you must provide us with the appropriate active claim number, allowed conditions, date of injury and mailing address of the processing office.
